CTRC Protocol Application Process

All HS-IRB and Minimal Risk IRB submissions (excluding Western IRB) must be sent electronically to . Applications not sent to that e-mail address will be returned for resubmission. The IRB will forward all CTRC applications to CTRC.

For CTRC review, the following documents must be included with your electronic IRB application and sponsor protocol/grant application. As noted above, the application is no longer submitted directly to the CTRC. Click here for submission instructions.

1. CTRC Application: DOC
2. CTRC Data and Safety Monitoring Plan Worksheet. For detailed information and forms click here
3. The IRB Initial Review application, with draft consent form(s), Information Sheet, HIPAA, and any other attachments provided to the IRB
4. The most current sponsor protocol/grant application.
5. Other pertinent materials [e.g., draft orders, PI biosketch (if a new investigator to the UW ICTR-CTRC), communications, RSP-approved budget, etc.]

These can all be submitted as separate "files", and do not have to be incorporated into one file. Adobe Acrobat/Reader (pdf) and Word (doc, rtf) files are recommended. The IRB naming convention for electronic files should be used in naming the files (e.g., BADGER-CTRC, BADGER-CTRCDSMP, BADGER-IRapp, BADGER-protocol, etc.)

Once you have submitted your application to the IRB and are directed to submit copies to the CTRC, please deliver the following to Danielle Gale (E5/610, CSC). This is separate from the copies required for the IRB review: five (5) stapled, double-sided copies (please staple all sections into one document, do not staple individual sections) of items 1-5, above, with signatures on the CTRC Application, DSMP Worksheet, and IRB Initial Review application.

The following are the CTRC Protocol Implementation Review dates for the remainder of 2009. Deadlines are 12:00 noon one week prior to the meeting. (Meeting and deadline dates are Thursdays, unless otherwise noted.) Please keep these dates in mind when preparing your application for submission to the IRB.
